Ideal Candidate Profile: Who Benefits Most from Thread Lifting?
Thread lifting is not a universal treatment applicable to everyone. Correct candidate selection is the primary determinant of treatment success. Ideal candidate characteristics:
- Ages 35–55: In this age group, skin still has sufficient elasticity and collagen reserves.
- Mild-to-moderate sagging (Jowl Grade 1–2): Threads provide mechanical lifting; they are insufficient for advanced sagging.
- Thin-to-medium skin thickness: Thick skin makes it harder for threads to grip and the result is more limited.
- Normal-to-mild fat distribution: Excess fatty tissue in the lower face can break thread resistance.
- Realistic expectations: Thread lifting is not a surgical facelift; it provides a natural, gradual lift.
- Individuals who decline or are too early for surgery: A safe alternative for those who do not want to take surgical risks.
Contraindications Table
| Contraindication | Explanation | Alternative |
|---|---|---|
| Active infection | Active infection at or around the treatment area | Wait until infection resolves |
| Autoimmune disease (active) | Lupus, scleroderma, RA in active flare | Assess during remission |
| Significantly overweight patient | Prominent facial fat excess breaks thread resistance | Facial lipolysis + filler |
| Advanced sagging (Grade 3+) | Threads insufficient; tissue weight prevents lifting | Surgical facelift |
| Anticoagulant therapy | Warfarin, heparin, NOACs | Decide with physician |
| Pregnancy/breastfeeding | Insufficient safety data | Plan for after this period |
| Keloid tendency | Risk of keloid at injection points | Careful evaluation |
Sagging Degree Assessment: The Jowl Grading System
The Jowl Grade system is used for objective evaluation of facial sagging:
- Grade 0: No sagging. Only filler or preventive procedures are recommended.
- Grade 1: Mild sagging, early jowl. Best candidate for thread lifting.
- Grade 2: Prominent jowl, moderate sagging. Thread + filler combination is recommended.
- Grade 3: Significant and advanced sagging. Threads may be insufficient; surgery should be evaluated.
- Grade 4: Advanced sagging. Surgery is required.
"Too Much Sagging for Threads" — When Is Surgery Necessary?
Knowing the limitations of thread lifting is critical for correctly guiding the patient. The following situations require surgical evaluation:
- Jowl sagging combined with prominent neck bands (platysmal bands).
- Excess skin in the lower face (skin laxity) — threads cannot remove excess skin, they only lift.
- Grade 3+ sagging combined with significant skin quality loss in patients over 60.
- Patients in whom previous thread lifting sessions have been insufficient.
Is There an Age Limit?
Theoretically, there is no definitive upper age limit for thread lifting. In practice, however, the following assessments should be made:
- In patients over 65, skin elasticity may have significantly decreased; the threads may achieve less grip.
- In this age group, PLLA or PCL threads may be preferred as they provide a longer-lasting effect.
- Combined approaches (threads + filler + Botox) may produce more effective results.
- Goals should be kept realistic: not "rejuvenation," but "halting and supporting."
Expanding Indications with Combined Treatments
Patients who initially appear to be "borderline" for threads can become good candidates with combined protocols:
- Excess fat + sagging: Facial lipolysis injection (deoxycholic acid) to reduce fat, followed by a more effective thread application.
- Thin skin + sagging: After improving skin quality with Skinbooster or Profhilo, threads can achieve a stronger grip.
- Predominantly collagen loss: The skin's biochemistry is first supported with PRP + mesotherapy, then threads are applied.
Why Is Thread Lifting Insufficient in Patients with Excess Facial Fat?
Body weight is not the determining factor — it is the amount of fatty tissue in the face. In patients with prominent fat excess in the lower face:
- Tissue weight strains the thread anchor points.
- Combined with gravity, thread anchoring gradually deteriorates.
- The aesthetic result may be lost in a short time.
For these patients, it is more rational to first plan a facial slimming treatment (buccal fat removal, lipolysis), followed by thread lifting. Please contact us for pricing information.
